Submitting the form constitutes express written consent to receive recurring SMS from us.
9 . Message Frequency
You will receive up to six (6) messages per month. Actual frequency may vary based on your appointments and interactions.

Message & data rates may apply according to your mobile carrier plan. Carriers are not liable for delayed or undelivered messages.
11 . Opt-Out

These Terms are governed by the laws of the State of Tennessee, without regard to conflict of law principles. Any dispute shall be resolved in the state or federal courts located in Knox County, TN.
